To start the application, you will need to gather some information related to your
practice. The following short list is the business infomation that you need.
- Copy of current California Insurance License
- E&O Insurance Declarations page (minimum limits of $1,000,000 is required)
- Copy of California Insurance Broker Bond
- Federal EIN or Personal SSN
Now you have gathered the information, you can download the broker agreement form,
fill it out at your convenience, sign and submit it along with the other documentation by
fax or email.
